The short answer

That combination is a reasonable case for a drug that targets appetite and insulin resistance simultaneously. It is also, as the next section explains, a reason to be unusually careful about what the weight loss is made of.
You are still fertile, and this is where people get caught out
Perimenopause means irregular ovulation, not absent ovulation. Cycles lengthen, skip, and return; a woman can go four months without a period and ovulate in the fifth. Contraception remains necessary until the accepted clinical thresholds are met, and that is a conversation for your clinician rather than an inference from your calendar.
Two GLP-1-specific factors sit on top of that. First, substantial weight loss and improved insulin sensitivity can restore more regular ovulation, so the drug may make you more fertile at precisely the point you had assumed the question was closing. Second, if you take tirzepatide and use an oral contraceptive, the label instructs you to switch to a non-oral method or add a barrier method for four weeks after starting and for four weeks after every dose increase, because tirzepatide meaningfully reduces oral contraceptive absorption. Semaglutide carries no equivalent warning.

Protect muscle and bone from the first week, not the last
Between 26% and 40% of the weight lost on semaglutide or tirzepatide is lean soft tissue rather than fat. In a 30-year-old that is a manageable cost. In perimenopause it lands on top of an age-related decline in lean mass and, critically, in the window when bone loss accelerates sharply around the final menstrual period.

Hormone therapy is a separate question worth asking early

What the evidence does not show
- No trial of GLP-1 drugs for perimenopausal weight gain specifically. The evidence is extrapolated from general obesity trials, in which perimenopausal women were a subgroup rather than the question.
- No evidence they treat perimenopausal symptoms. Hot flushes, sleep disruption, and mood are not GLP-1 targets and should not be expected to respond.
- No randomised trial of hormone therapy plus a GLP-1 drug, in any menopausal stage.
- No long-term bone outcome data. Bone density is a surrogate; nobody has run these drugs long enough in this population to measure fractures.
